A practical exercise is designed to understand how to calculate safe and at-risk behavior at the workplace. You can choose any activity from your workplace and develop a checklist for observation. A sample checklist is attached for your reference along with the formula on how to calculate safe behavior.
Below is a sample behavior observation checklist for carrying out electrical maintenance work. Check the complete procedure in the next topic and perform the exercise at your workplace to get hands-on experience.
Number of observation done = 4
| Behavior | Yes (Number) | No (Number) |
| Wearing the necessary PPE for the electrical task | 3 | 1 |
| Using lock-out/tag-out prior to performing maintenance | 2 | 2 |
| Shutting off the supply before removing the panel | 1 | 0 |
| Remove the faulty part using both hands | 4 | 3 |
| Total | 10 | 6 |
Once the observations are complete, calculate the percentage frequency of the safe acts. You can do this for a specific department or a specific task.
Calculation of % safe acts = Number safe acts/ Number of safe acts + number unsafe acts
Calculation of % safe acts = 10/ 10+5
Calculation of % safe acts = 10/ 15
Calculation of % safe acts = 0.67
